Please complete the Solicitation Permit application in it's entirety. It is also your responsibility to obtain a Pennsylvania Criminal Records check through the Pennsylvania State Police and submit it with your application. The records check request form is included with the Solicitation Permit application for your convenience. When completed, you can mail to or drop off the forms at:
