Identity Theft Checklist for Victims

File a Police report with your local police department, immediately. 
Obtain a copy of your credit report.  You are entitled to one free report per year.  Go towww.annualcreditreport.com to request your copy.  You will need to provide your social security number in this case.  
Obtain and complete an Identity Theft Affidavit for each compromised account.  
Close all accounts that you believe have been compromised or opened fraudulently.  
Place a fraud alert with each credit bureau

Block all fraudulent information from credit report. 
Keep a detailed journal of all contact that is made concerning the theft of your identity, including names and phone numbers.  Forward all copies of any statements or information to your local police department.  
Continue to monitor your credit report for any discrepancies.
File a complaint with the Federal Trade Commission at 1-877-438-4338 or www.consumer.gov/idtheft.  
Complete an Identity Theft File Consent Document, if you meet criteria.  Select your 1-8 digit password and provide it to the Officer completing the document. 

Place a lock on your credit report.  This service is free to victims of ID theft.  It prohibits anyone to pull their credit report to open new accounts or make large purchases without them personally unlocking their file.
